The culture adjustment of heart to heart card and its application in patients with advanced cancer

Objective To revise the heart to heart card and explore its application in patients with advanced cancer. Methods From July to September 2022,Delphi technology and cognitive interview method were adopted to conduct cultural adjustment of heart to heart card. Convenience sampling method was adopted to select advanced cancer patients who were hospitalized in a tertiary cancer hospital in Hubei Province in October 2022 as research objects,and feedback on the application of the heart to heart card was collected. Results The effective recovery rate of the 2 rounds of expert correspondence questionnaires was 100%;the expert authority coefficients were 0.92;the Kendall harmony coefficients were 0.215 and 0.100,respectively. The coefficient of variation of each item in the second round of correspondence is 0~0.186. Combined with the results of 3 rounds of cognitive interviews,the heart to heart card consists of 54 items in 4 dimensions,including physical needs,social needs,psychological needs and spiritual needs. The application effect is good in 15 patients with advanced cancer. Conclusion The culturally adjusted heart to heart card is scientific and practical,and can be used as a tool to assess the needs of patients with advanced cancer.

Objective To systematically review and synthesize the experience of informal caregivers caring for patients with chronic heart failure at home. Methods 9 databases,including Web of science,PubMed,Embase,The Cochrane Library,EBSCO,PsyclNFO,Sinomed,CNKI and Wanfang Data,were searched from their inception to May 2021 for the qualitative studies on experience of informal caregivers caring for patients with chronic heart failure at home. The study quality was evaluated using the qualitative research quality evaluation criteria of the Joanna Briggs Institute Evidence-based Health Care Center in Australia. The research results were summarized and integrated using the pooled integration method. Results A total of 15 articles were included and 38 complete findings were grouped into 10 categories according to their similarities. These 10 categories resulted in 3 synthesized findings:the change of the normal life(challenge,stress,and conflict),the lack of external support(knowledge,resources,and expectations),the gain of positive feelings(identification,benefits,growth). Conclusion Nurses should pay more attention to the burden,stress,as well as needs of heart failure caregivers,improve their caregiving competency,and give professional support and emotional support to them. By these ways,quality of life of patients with heart failure can be ultimately improved.

Evidence summary of perioperative health education for patients undergoing ostomy surgery

Objective To retrieve,evaluate and summarize the best evidence of perioperative health education for patients undergoing ostomy surgery.Methods We searched Registered Nurses' Association of Ontario,Scottish Intercollegiate Guidelines Network,New Zealand Guidelines Group,Australian Government National Health and Medical Research Council,Guidelines International Network,BMJ Best Practice,UpToDate,Joanna Briggs Institute library,Cochrane library,and special websites,such as World Council of Enterostomal Therapists,Wound,Ostomy and Continence Society,American Society of Colon and Rectal Surgeons,Nurses Specialized in Wound,Ostomy and Continence Canada,United Ostomy Associations of America,American Urological Association and European Associations of Urology,to collect relevant guidelines,evidence summaries,recommended practices and systematic reviews and expert consensuses published from database construction to January 7st of 2020. 2 researchers evaluated the quality of the literature,and extracted recommendations and research conclusions from the included literature. Results Totally 11 articles were involved,including 5 guidelines,3 evidence summaries,1 recommended practice and 2 systematic reviews. Finally,18 pieces of best evidence about educator qualification,education targets,education timing,assessment,preoperative education,postoperative education,education strategy and evaluation were summarized.Conclusion This study summarized the best evidence of perioperative health education for patients undergoing ostomy surgery. With reasonable planning,it can reduce the incidence of stomal and peristomal complications,promote patients to return to society as soon as possible,and thus improve the quality of life of patients.

Study on the adaptability of personalized drinking water nomogram to the disabled elderly in nursing homes

Objective To explore the adaptability and application effect of personalized drinking water recommendation nomogram in the elderly with disability in nursing homes,and to provide references for the prevention of dehydration of the elderly with disability in the nursing homes. Methods In October 2018,95 disabled elderly people with normal plasma osmolality(280~<295mmol/L) were selected from a nursing home to investigate the 7-day drinking water consumption,and the differences between the recommended drinking water consumption of personalized drinking water consumption recommendation nomogram and the average 7-day drinking water consumption of the elderly were analyzed. From March to August 2019,150 cases of the disabled elderly in the nursing homes were randomly divided into a test group,control group 1 and control group 2. The test group received the recommended drinking water quantity of personalized drinking water quantity recommendation nomogram;there was no intervention in control group 1,and the disabled elderly drank according to the original amount of drinking water and habits;control group 2 received 1 200 ml/day. The plasma osmolality,constipation and urinary tract infection of 3 groups were compared before and 6 months after the interventions. Results There was no significant difference between the recommended drinking water quantity of the personalized drinking water quantity recommendation nomogram and the 7-day drinking water quantity mean value of the normal plasma osmotic pressure elderly(P>0.05);6 months after the implementation of different drinking water targets and drinking water interventions,the differences of plasma osmolality between 3 groups were statistically significant(P<0.001). The comparison between 2 groups showed that the intervention effect of the test group and the control group 2 was better than that of the control group 1,and the intervention effect of the test group was better than that of the control group 2. Conclusion Considering the individual differences of the disabled elderly,personalized recommended drinking water nomogram is suitable for the disabled elderly in nursing homes and it can improve plasma osmotic pressure.

Establishment and application of PICC information management platform for intravenous treatment league

Objective To establish and apply the PICC information management platform for intravenous treatment league to achieve PICC continuity care. Methods We designed the functions of PICC information platform in accordance with the requirements of “Internet + Nursing Services”. We designed a simplified model of the PICC information platform based on the Freeman 2007. We established and applied the PICC information management platform for Chongqing intravenous treatment league,and evaluated the effects. Results The PICC information platform consisted of management center (PC) and mobile phone client end(APP)for nurses and patients. The nurses end contained 13 modules in 4 sections,and the patients end contained 10 modules in 5 sections. The platform was able to collect the whole-process data of PICC insertion,maintenance,and extubation,as well as management data such as consulting,consultation,and follow-up generated during the process,realizing continuity of PICC care process. Untill May 31 st,2019,the platform has been used for 2.5 months,with 81 registered hospitals,1595 registered medical staff,and 457 patients. The amount of data reported by tertiary hospitals in the platform was significantly higher than that by other hospitals. Conclusion The PICC information management platform provides closed-loop management of PICC continuity care,enhances the initiative of PICC patients’ self-management,and supports the exploration of “online + offline” specialist care models.

Objective To retrieve,evaluate and summarize the best evidence of pelvic floor muscle training applied in prevention and treatment of post-prostatectomy incontinence,so as to provide references for clinical practice. Methods We searched the guideline websites,association official websites and databases to collect literature including guidelines,evidence summaries,best practice information sheets,recommend practice and system reviews. 2 researchers independently evaluated quality of literature and extracted data. Results Totally 11 articles were enrolled,including 3 guidelines,1 evidence summary and 7 systematic reviews. Finally 11 pieces of best evidence on training aims,assessment of urinary incontinence,application timing,effects and methods,factors influencing effects of interventions were summarized. Conclusion This study summarized the best evidence of pelvic floor muscle training applied in prevention and treatment of post-prostatectomy incontinence to provide more targeted and scientific guidance for clinical medical staff in the assessment of urinary incontinence,pelvic floor muscle training,health education and instructions for patients undergoing radical prostatectomy.

Research on the construction of training knowledge system to prevent inpatient suicide

Objective To construct the knowledge system of inpatient suicide prevention training and provide bases for clinical nurses to carry out suicide training in inpatients. Methods Based on literature review,the training knowledge demand status of nurses for patients to prevent suicide was surveyed,and the expert consultation draft was preliminarily constructed. Delphi method was used to organize the recommendations from 25 experts that specialized in suicide prevention,nursing safety management,nursing humanistic care and psychological nursing. In accordance with 2 rounds of expert consultations,some items were revised. Results The response rates of 2 rounds are respectively 96.0% and 91.7%. The authority coefficient of experts is 0.88 and 0.83 respectively. The Kendall harmony coefficient was 0.29 and 0.17 respectively(P<0.001).The final knowledge system of patient suicide prevention training consists of 4 first-level items and 26 second-level items. Conclusion The knowledge system of inpatient suicide prevention training has high reliability and practicability,which can provide bases for clinical patient suicide prevention training.
